As we age, we can expect to experience memory loss, decreased attention span, inability to reason or problem solve, degradation of our senses, or other very necessary brain functions that many of us take for granted. Aging SMART programs are designed to stimulate and revitalize these neurological abilities, restoring our clients’ confidence and improving their quality of life.
Geriatraic Care Management
We provide assessment, organization, and monitoring services to help individuals navigate through a system that can seem fragmented and puzzling for many senior adults. We assess each client’s problems and offer personalized service options to assist individuals and caregivers in dealing with financial and medical demands, and help establish short and long-term care plans.
Who Benefits from Aging SMART Services?
This program is specifically designed to stimulate brain function for the active senior, those with mild cognitive impairment, or early stage Alzheimer’s Disease and Dementia by employing a variety of cognitive challenging activities. All activities focus on cognitive strengthening, maintaining and improving hand eye coordination, and using fine motor skills.
