Stacey Zaslow-Cook, LMSW, C-ASWCM
Stacey has worked as a geriatric social worker for over 30 years, and for the past 2 years, she has been the owner of Aging Smart. She earned a BS degree from Rochester Institute of Technology and her MSW from New York University. Stacey is a member of The National Association of Geriatric Care Managers and is a licensed social worker. Stacey worked at different hospitals as a geriatric social worker, coordinating safe discharges into the community, such as nursing homes, rehabilitation centers, and adult day cares. Stacey has worked in a medical adult day program and a homecare agency assisting clients in obtaining community entitlements, running a support group for seniors to adjust to new life changes and to ensure safety in their homes.
Stacey started a caregiver’s support group at the facility and has given numerous presentations on senior housings, including a presentation for The Department of Aging entitled, “Navigating the Health System: Finding the Right Service.”Stacey also led a round table discussion on caring for the elderly and demented elderly at the Department of Aging’s 20th Mayoral conference on Alzheimer’s disease.
